(7) APPOINTMENT CANCELLATION & RESCHEDULING POLICY

In accordance with the Program’s Services, Student will have one-on-one appointment(s) with the Company’s Health Coach. Company requires at least 48 hours advance notice prior to cancellation by Student of any scheduled appointment. If Student misses his/her scheduled appointment, or cancels with less than the above advance notice, Student forfeits Student’s appointment and shall not be entitled to any refund or compensation for cancelling the appointment with less than the above advance notice or for a no-show. All rescheduled appointments shall be rescheduled within the recommended Program’s time frame. If Student misses his/her scheduled appointment, or cancels with less than the above advance notice due to an emergency, illness, or event beyond Student’s control, Student shall communicate this immediately to Student’s Health Coach or applicable Company representative.

(8)  SUPPLEMENT SALES ARE FINAL

All supplement sales are final. There will be no refund given for supplements. Any attempt to return supplements to the distributor or Company will have no effect on this refund policy.

I, the Student to this Agreement, understand and acknowledge the above information regarding the appointment cancelland and rescheduling policy and that all supplement sales are final. I also acknowledge that I understand this and consent to participate in the Program. I understand this is Page Five of this Agreement and there are seven pages to this Agreement.

I HAVE CAREFULLY READ THIS AGREEMENT AND ACKNOWLEDGE THAT I UNDERSTAND IT.

By using your electronic signature, you, the Student, acknowledge and agree that it carries the same legal weight and validity as your handwritten signature. It serves as your authentic and binding endorsement and agreement on this electronic document, just as if you had physically signed this Agreement with pen and paper.
